Community Transport

Cabonne Community Transport Service is a non government community based scheme, accountable to the Ageing and Disability Department and Transport for NSW.

This service is funded under the Community Home Support Program (CHSP), Community Transport Program (CTP) and Western NSW Local Heath District.

The CHSP program caters for eligible frail aged people with moderate, severe, or profound disability, and their carers who are living in their own homes and the CTP program caters for the transport of eligible disadvantaged people who have no other form of transport living in their own homes.

Transport services are provided to assist in maintaining independence for those who are transport disadvantaged to appointments or social outings and Western NSW Health Program caters for residents to attend hospital or medical appointments referred by medical professionals.

Community Transport does not transport clients who are in cared facilities such as nursing homes or hostels or clients who already access aged care packages at home. It will not transport clients to employment or education facilities.

Bus leaves Yeoval Memorial Hall at 9.15am, Cumnock Post Office at 9.30am, Molong Yarn Market at 9.45am. Returns to Yeoval at 2.30pm

What is Community Transport?

Community Transport is a scheme that provides safe, easy to access, comfortable transport for people who have difficulty in accessing mainstream transport, including the frail aged, people with disabilities and their carers. This scheme is managed by the Cabonne Council and the services provided by volunteer drivers.

The aim of the service is to offer a door to door service that allows you to keep your independence and continue to lead a life you want to live and enable people to remain living in their own homes.

How much does it cost?

The cost of the trip depends on what service you are using and the distance that you have to travel. Costs are reasonable and these contributions assist the service in providing a valuable service.

What is Health Related Transport?

The Health Related Transport Service aims to provide a quality, cost effective transport service for people needing assistance with transport to and from health services, providing these people meet the eligibility criteria for the service.

The scheme is funded by Western NSW Local Health District and managed by Cabonne Council and is provided by volunteer drivers.

How much does it cost?

The cost varies with the length of the trip and is set at about onr third of the cost to the service. The other two thirds of the cost is borne by the service. Although Health Related Transport does incur GST.

The coordinator can tell you about the costs if required.

How do I get the service?

You must be referred to this service by a Medical Practitioner or Staff member of the Area Health Service with clinical qualifications (e.g. Nurse Unit Manager, Discharge Planner, Community Nurse.

Bus outings to attractions in the local region and other areas are organised at various times during the year.

Details of the latest social trips, which range from one day to several days, will be displayed on this website or you can phone the office for information on upcoming social outings.

There is an eligibility requirement for these trips.
